ABSTRACT: Biliary atresia is a rare disease of infancy with a relatively unknown etiology. We report a case of premature, 27-week monozygotic twins who are discordant for the development of biliary atresia. We systematically reviewed the literature, and found 30 cases of twins with biliary atresia; only two sets were concordant for the disease. The case was complicated by a delay in diagnosis related to the prematurity of the patient and a comparison to the twin's clinical course. This case supports the theory that the etiology of biliary atresia is related to epigenetic modification.
